This form is for the exclusive use of travel agents for individual reservations made with Transat Tours Canada via the Customer Contact Center, Sirev or Revnet.

Requests to add Option Flex and date of birth corrections are not possible within 21 days of departure.

The same Option Flex must be added for all passengers in the same room.

By submitting this request, I confirm that I have read and understood all the conditions described above. I also understand that it is my sole responsibility to explain these terms to my clients and obtain their consent. The addition of Option Flex must be requested a maximum of 2 days following booking for a departure of more than 21 days. Agents must apply the payment themselves. A request to withdraw Option Flex from an optional file must be made at the time the payment guarantee is applied. Date of birth corrections are only possible for an adult (18+) for a departure of more than 21 days. Only notes that are not available for self-service in Sirev will be added. Eco Fares cannot be modified via this form.
